Deaconess HomeCare’s Telehealth Program provides patient monitoring, education on self-management behaviors, vital sign monitoring and daily contact with the healthcare team. This patient monitoring is accomplished through the use of a small, easy-to-use, in-home communication device called the Health Buddy. All information is kept confidential. Health Buddy: The Health Buddy allows the healthcare team to be informed about the patient’s condition on a daily basis and therefore provides the physician with timely updates. Important clinical symptoms are monitored and educational components are delivered on a daily basis. The Health Buddy also provides helpful facts and useful self-care tips our patients can use to lead a healthier life.
