An evaluation will be done by a nurse or therapist to assess the specific needs. A plan of care is developed with the MD, DHC team and family to determine the best course of treatment for the patient. Conditions that might indicate a need for home care- Frequent Emergency Room visits
- Recent weight loss or gain
- Frequent re-hospitalizations
- Changes in ability to walk
- Change in current diagnosis
- Changes in ability to swallow or speak
- Frequent falls or dizziness teaching, assessment and observation
